- two uncertified copies of government-issued official identification document (e.g. national ID card/passport, national ID card /driver license, passport/driver license).
If you are residing in other countries (non Polish citizen) or foreigners residing in Poland without identification document issued by the Polish government, please attach:
- a certified copy of government-issued official identification document in your country (e.g. national ID card/passport).
If you are a Norwegian citizen, proof of your citizenship is required.
